As a healthcare provider, Maven In Behavior Inc. is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your child's protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the notice currently in effect.

The complete notice describes, in detail, how we may use and disclose PHI for treatment, payment, and healthcare operations; the situations where the law permits or requires other disclosures; and every use that requires your written authorization.

Your rights over your child’s health information

Under HIPAA you have the right to: inspect and receive a copy of the health record; request corrections; request restrictions on certain uses and disclosures; request confidential communications (for example, calling only a certain number); receive an accounting of certain disclosures; receive a paper copy of the full notice on request; and file a complaint — with us or with the U.S. Department of Health and Human Services, Office for Civil Rights — without any retaliation.

Our commitments

We use and disclose the minimum necessary information for the purpose at hand. We train our team on privacy. We do not sell health information, and we do not use it for marketing without authorization. If a breach of unsecured PHI occurs, we will notify you as required by federal law (HIPAA/HITECH) and Florida law (Fla. Stat. § 501.171).
